Multi-Modal Treatment Planning For Natural Looking Aesthetic Rejuvenation (Or how we get it right! )


Anti-wrinkle injections weakens muscles, we know that.  But ageing is multifactorial, not just a product of muscle over activity.  To create natural looking aesthetic rejuvenation it stands to reason that we need more than just anti-wrinkle injections to keep us looking younger.  Obvious isn’t it!  Yet we still find that anti-wrinkle injections is the worlds number one requested cosmetic treatment.

Anti-Wrinkle Injections can relax the lines and wrinkles on my forehead, if I had any there.  It can also smooth lines and wrinkles around my eyes and between my eyebrows, likewise if there were any there. However, it’s not going to help overall with making me look and feel a bit younger.  In fact, if I had nothing but anti-wrinkle injections for the next ten years I’d start to look decidedly odd! With a smooth and wrinkle free upper third of my face but quite an aged appearance to everything else.

What I am seeing when I look at this image is the deeper shadow developing at the side of my nose, the hollowing at the cheek at the side of my mouth, the wrinkling of the skin underneath my eye and the flattening of my cheek underneath the eye. If I was being super critical I’d probably also mention some very early volume loss just above my eyebrow that will probably develop over time and make my eyes look a bit more skeletal.  For the sake of a complete picture I can also see a receded hairline and marked thinning of the hair on the top of my head.  That’s what more than ten years in Aesthetic Medicine will do to you!

The answer is clearly not just anti-wrinkle injections.

Let’s think about a real person now, a typical prospective aesthetic patient who presents to a clinic just like Clinetix wanting to look and feel refreshed and rejuvenated.  Lets assume it’s a female patient right in the middle of the age range of people who attend my clinic about 45.  When she sits down in front of me and tells me she feels her appearance makes her look and feel tired or depressed I don’t reach immediately for the wrinkle syringe, I reach for the mirror and ask her to point to what is causing her concern.

These days it’s getting less likely for the fingers to be pointing at the crows feet, or the frown lines. Instead they generally go to the middle and lower parts of the face first, indicating sagging skin, jowl formation, downturned mouths with thin lips and changes in skin quality like roughness, pigmentation or large pores.  We consider these features as part of a holistic rejuvenation process in order to create better results for our patients.

Creating Natural Looking Aesthetic Rejuvenation with a Multi-Modal Treatment Plan

As ageing is multifactorial we approach rejuvenation with a multi-modal treatment plan.  This means we can use a wide variety of treatments to address individual aspects of ageing together to create a synergistic and harmonious natural looking rejuvenation.

The Volumetric Approach To Facial Rejuvenation

I like to visualise the ageing face from the bottom up.  Think of an old house that’s sitting on dodgy foundations.  The roof is falling apart and the window frames are all broken.  I can fix those things easily but the house will still be sinking on it’s foundations. Therefore there’s not a huge amount of point other than for the temporary cosmetic fix.  For a long term fix you need to start with the foundations to build a house that will last.

The foundation of the face is the bones of the skull and the dense layers of fat that sit above.  As we age we get specific changes in the shape of the skeleton, areas that flatten and regress.  We also get reduction in the amount of dense fat that overlies it.  Most of my consultations begin with an assessment of the foundations of the face and a plan to replace that lost volume.  The volumetric approach to facial rejuvenation.  With a detailed understanding of the anatomical changes of the skeleton with ageing, and the availability of safe injectable implants (dermal fillers) we create a plan to inject in specific areas to restore a natural shape to the deep face.

This will build up a strong support upon which the upper layers of the face can be more naturally restructured. The amount of dermal filler required to achieve such a correction varies with age of the patient. However it is usually fairly predictable using the formula 1cc per decade of life plus 1cc.  This means that someone in their mid forties will usually require 5 syringes, someone in their mid fifties may require 6 and someone in their mid thirties may require 4.  We place the dermal filler in the areas of the face to give maximum support as required for the patient. Typically these areas are the cheeks, the temples, the side of the nose and the jawline.

The next area of concern is the more superficial fat layers of the face, these can be smoothed with softer, less lifting dermal fillers to correct irregularities and improve contour. But when sagging of the skin and early jowl formation become a concern then we often need to think in terms of re suspending the user layers.  The traditional approach to achieve this is the surgical facelift. However, the advent of safe and effective thread lifting techniques now gives us a non-surgical alternative.

Threads are introduced through small holes made with a needle at the side of the face and placed into the upper layers of the faced.  Using a thread called a PDO Cog, we can place a fan of threads that can be gently pulled. This creates a lift and repositions the upper layers of the face.  This technique is very effective at lifting the upper fat layers of the face. Which in turn has an effect on the appearance of nasolabial lines, marionette lines and jowls.

Rejuvenating the Appearance of the Skin

Once we have revolumised and lifted the face the final thing to consider is the skin.  There are many many different ways to rejuvenate the appearance of the skin and the decision will depend on the desired result, the budget and the desires of the patient to have a quick result with one or two treatments, or a more gradual process with several skin treatments over a period of time.  With our non-surgical rejuvenation packages we offer CO2 Laser resurfacing, PRP treatments or Chemical Peels to get rapid and dramatic results.  We also have a range of specially designed combination skin treatments that treat multiple layers of the skin using different techniques at the same time for improved results with no downtime.

Laser Resurfacing

Laser resurfacing or Fractional Ablative resurfacing uses a high powered laser to cause controlled damage to the skin at a specific depth. The skin responds by creating more healthy collagen and more ground substance in the dermis. This results in firmer and more resilient skin.  The epidermis is also resurfaced providing a fresher and more lustrous skin with less blemishes, pigmentation and surface irregularities. The recovery period following a laser treatment is about a week.  Initially your skin will be red and it is likely to take some time to heal afterwards.

Platelet Rich Plasma

Natural Looking Aesthetic Rejuvenation with Vampire Face Lift, PRP, What Aesthetic Treatment Is Right For Me at Clinetix Rejuvenation?You may have heard PRP called the Vampire facelift.  This treatment involves taking a sample of the patients blood. We then treat the blood in a centrifuge to remove the red and white blood cells, leaving behind a mixture of plasma and platelets.  It is the platelets that are important as we can reinject this fluid back into the skin. These platelets are responsible for wound regeneration and repairing processes that results in a healthier smoother skin. Typically there is no downtime following the Platelet Rich Plasma procedure but we recommend three treatments for the best results.

Chemical Peels

This is a solution that is applied skin and allowed to soak through the deeper layers of your skin.  The TCA causes a controlled damage to the dermis at a specific layer. The skin above this layer then peels off over the course of a week as healthier fresher skin grows underneath.

To summarise, a complete facial rejuvenation treatment plan should consider all the signs of ageing. It should also be able to address each one separately in order to achieve a natural aesthetic result.

Our goal is to make our patients feel better about the way they look. In turn make them feel more confident about themselves as a whole!  The goal is never to make someone look like they have had a treatment.  After a treatment people should be complimenting you about your appearance, but they won’t know what has changed.  As I often say to my patients, if anyone asks you who your Doctor is, come and tell me because it means I’ve done something wrong.  If they ask you if you’ve been on holiday or changed you hair then we’ve got it right!

What Treatment is Right for Me? (Part II)


In a previous blog I began to talk about the different type of aesthetic treatments we offer at Clinetix Rejuvenation and how they work. In this one I’m going to take about a few more treatments that we offer at our clinics in Glasgow. The aesthetic treatment we start to discuss is PRP, also known as the “Vampire Face Lift”

Platelet Rich Plasma (PRP)

Aesthetic Treatment - PRP, Vampire Face LiftPlatelet Rich Plasma and is a treatment that we use frequently for generalised skin rejuvenation. It is effective in improving skin hydration and lustre, as well as improving fine lines and wrinkles.

At Clinetix we frequently combine PRP with other skin rejuvenating treatments such as phototherapy, micro needling and laser resurfacing to improve and accelerate the results. PRP was originally developed as a treatment to help wound healing in surgery. Platelet Rich Plasma has now spread to being used in cosmetic practice as it’s results became apparent. PRP is a fluid that is made from the patients own blood. The process involves taking a blood sample and using a centrifuge to separate out the various components. The Red Blood Cells and the White Blood Cells are discarded. Our medical team use the remaining yellowish clear fluid which is called Plasma, and contains a high concentration of cells called Platelets. Platelets are part of the bodies regeneration and repair system and contain a large number of molecules that help promote the rejuvenation of tissues including skin.

The PRP fluid is re-injected into the skin to be treated in various depths. This can be done in a variety of different ways and we are currently using a device called a “meso-gun”. We choose to adopt this technique as the meso-gun allows the treatment to be performed much faster and with no discomfort at all. Treatments typically take less than an hour and although there is usually no down time. Our patients see an immediate improvement in skin hydration after the treatment and the results continually improve over several months. You can repeat treatments as necessary and we usually provide a course of at least 3 treatments 3 weeks apart for maximum results.

Thread Lift Treatments

Aesthetic Treatment - Thread LiftThread lifts are becoming increasingly popular as the development of better threads and better techniques are providing impressive and long lasting results. These thread lifts are designed to lift and reposition the upper layers of the face. Thread lifts are ideal for people who are starting to develop skin laxity with nasolabial folds, flattened cheeks and early jowl formation. Threads can be used in the lower part of the face and the upper neck to help define a jawline.

The threads themselves have either small barbs or cones that anchor onto the tissues and allow the thread to be gently tensioned, resulting in a lift of the skin. The process is performed using local anaesthetic injections to numb small areas of the face. We introduce threads under the skin using needles or cannula in the desired area and are tension the threads to lift before being cut. After they are cut the thread then becomes completely buried under the skin and is invisible. The results generally last for 18 months or so and can be prolonged if smaller treatments, involving less threads are repeated every year or so.

Laser Resurfacing

Aesthetic Treatment - CO2RE Laser ResurfacingFinally for today I’m going to cover Laser resurfacing. This is the Gold Standard treatment for photo aged skin that addresses texture, lines and wrinkles and laxity. We use a technique called Fractional Ablative Resurfacing with a Carbon Dioxide Laser in Clinetix. After numbing the skin with anaesthetic injections we use the laser to make a controlled injury to the epidermis and dermis, this results in the skin regenerating with a healthier dermis and epidermis. This is described as a downtime procedure that requires a little bit of aftercare. The skin on your face will take about a week or so to return to normal but the results are usually well worth the effort!

People are often concerned that this treatment may be painful but using good anaesthetic techniques most patients find the procedure remarkable tolerable. We combine Laser resurfacing with phototherapy and PRP treatment as this reduces the recovery time and improves the overall result. Post operative care is especially important following Laser Resurfacing. Patients are provided with information on how to clean and care for the skin until it heals. The Clinetix Thread Lift

Do I need a thread lift? – be honest with me doctor!

This is a very frequently asked question from patients in Clinetix and is actually quite easy to answer, at least to start with.  The first thing I point out is that practically no one actually needs a thread lift, or a face lift, or any other aesthetic procedure.  It’s an odd thing to hear from a cosmetic doctor running a successful practice but there you go.

The question that will have a more satisfying answer is not do I need, but do I want.  Do I want a thread lift?  (This applies to any cosmetic medical procedure) ask yourself will this thread lift (or any other procedure) make me feel more comfortable with my appearance, improve my confidence and help my self esteem?

Lets work on the assumption that you are concerned about your appearance and a procedure to make you look more youthful would be of benefit to you.

So back to the thread lift, what is a thread lift and what can it do?

Thread lifting techniques for lifting loose and sagging facial skin have been around for several decades, as surgeons and patients both try to innovate safer, less invasive and more effective treatments.  The basic principle is to insert a suture or thread through the skin around the temple area, anchor it to the loose and sagging skin and fat of the midface, and use the suture as an anchor to pull and reposition the skin.  The suture can be either attached to the deep fascias of the face, or left buried under the skin in the more superficial subcutaneous layers.  The older surgical approaches required deep anchoring and a small incision to be made however a number of newer techniques have evolved that do not require deep sutures and allow the procedures to be performed through small needle entry points.

As with any new surgical or medical innovation there were initially concerns and scepticism, as the sutures initially used had a chance of extrusion or chronic inflammation, and the techniques themselves produced less satisfying results. There has been a recent resurgence of the popularity of minimally invasive office based thread lifting procedures in the last five years with the development of two main devices.  One device called the silhouette soft thread lift uses threads with a small number of cones along it’s length designed to pull on and anchor loose skin.  The other system uses a finer thread with thousands of much smaller barbs along it’s length.

The Clinetix Face Lifting Approach

For mild to moderate sagging skin and ageing an option is the Clinetix Thread Lift.  This is a a treatment for mild to moderate facial sagging and consists of a bespoke PDO thread lift to reposition the facial skin, as well as a thread treatment to stimulate collagen and skin tightening around the jaw line for added longer lasting benefit.

The Clinetix Thread lift is the cornerstone of our non surgical face lifting procedures.  Using a combination of techniques learned from a variety of international experts together with specially selected stitches we are getting effective and long lasting results.  The treatment is performed in the clinic and typically takes about an hour.  There are some small injections for local anaesthetic but the treatment is generally painless. There may be some bruising afterwards that can last for up to a couple of weeks although most people have only slight bruising that is easily covered with make up.

For patients with loose sagging skin as well as facial volume loss the Clinetix Volume Replacement Face Lift consists of volume replacement with dermal fillers to strategic areas as well as thread lifting. The dermal fillers are placed in areas of deep facial volume loss such as the cheeks, the jaw line and the temples to provide a deeper underlying lift and a more natural rejuvenated appearance in patients with mid facial volume loss.

For patients who require skin improvement in addition to volume replacement and threads the Clinetix Complete Non-Surgical Face Lift combines all of the above with CO2 Laser Skin Resurfacing and PRP to smooth out, tighten and rejuvenate the skin.

The addition of Laser skin resurfacing means that the face can be rejuvenated in three levels.  The deep layers have volume replacement, the middle layers and the skin benefit from being repositioned by the threads and the laser resurfacing with PRP rejuvenates and tightens the skin.

PRP Treatments at Clinetix


What is PRP?

PRP or “Platelet Rich Plasma” is a solution that is created from a sample of your own blood. Blood contains many components, different types of cells, suspended in a liquid solution. If we take a small blood sample it can be treated in a process called centrifugation to separate out the components leaving us with a solution of fluid containing high concentrations of cells called platelets.

What does PRP do?

The platelets in your blood are responsible for regeneration and repair of your tissues. When you have an cut or a scrape, the platelets in the area will rupture, releasing a potent mix of molecules that act as signals to other cells and co-ordinate the repair process. We can use the solution of platelet rich plasma to stimulate a repair and regeneration process in the skin that will result in improved skin quality.

How is it used?

It can be used in a few different ways in aesthetic medicine, the simplest way is to take the solution and use a technique called microinjection to place it directly into the skin. This is sometimes called the “Vampire Facelift” and is one of the most popular forms of PRP treatments. A series of three or more sessions are usually recommended and the regeneration process in the skin continues for several months after treatment. It is also used in combination with other skin rejuvenation treatments such as laser resurfacing and micro-needling. These treatments regenerate the skin by creating a controlled injury with subsequent healing. PRP is often used in combination with these treatments to result in an improved outcome and a faster healing process.
